This legal disclosure of a massage therapist/Complimentary and Alternative Medicine Practitioner is not required under current Florida Law, but is provided with the anticipation that it may be in the future. It also provides certain information important in your decision for giving Informed Consent for massage therapy.

Licensure Disclosure: I am a licensed massage therapist, licensed by Florida’s Dept. of Health under the Division of Medical Quality Assurance. My Florida massage therapist license number is
MA38170
. I have no disciplinary actions taken against my massage license. Of course, I am properly licensed by the City of West Melbourne, FL and Brevard County, FL for the practice of a professional occupation within their jurisdictional boundaries.
My National Provider Identifier is 1558464768. I am an out-of-network Blue Cross/Blue Shield Provider (#C2165 FL).
I only offer massage services in my office at the Natural Approach Acupuncture & Massage Center facility. I do not travel or do "out calls" to your home or hotel.
My title is Florida Licensed Massage Therapist, although, some may call me a bodyworker. Please do not call me a "masseuse" or "masseur", nor use the term "massage palour" which are codewords for illicit activity in organized crime in Florida.
Education Disclosure: I hold a Master's of Science degree in Occupational Therapy from Barry University in Miami. I additionally hold a Bachelor’s of Science degree in Marine Transportation from Texas A&M University at Galveston. I hold a

Practice Disclosure: I practice (for compensation) massage therapy ("Massage" means the manipulation of the soft tissues of the human body with the hand, foot, arm, or elbow, whether or not such manipulation is aided by hydrotherapy, including colonic irrigation, or thermal therapy; any electrical or mechanical device; or the
application to the human body of a chemical or herbal preparation).
I am
certified in Neuromuscular Therapy
, which is the primary Frame of Reference which guides my clinical practice model. This is a deep tissue modality . Neuromuscular Therapy can be painful, but I will try to maintain a tolerable pressure with you at all times.
However, it is not the only Frame of Reference (F.O.R.) I use, and will change F.O.R's guided by what you need. I sometimes use other techniques, such as Swedish Massage (see below).
Post-massage soreness is typical of deep-tissue massage techniques. This is particularly true the day following treatment. If this is a problem, I recommend you not proceed with the massage, or specifically request Swedish style massage. I will give you a cold pack with instructions during your first visit to help prevent post-massage soreness. It is your responsibility to use your cold pack. Please do so.
I do not practice the techniques of: reflexology, CrainoSacral therapy, quantum touch, reiki, anma, hot stone massage or shiatsu, though I do incorporate a few techniques from myofascial release, thai massage, trigger point therapy, NDT, PNF and some physical agent modalities(hot/cold packs, E-Stim & cold laser) as allowed by Florida Statute.
Scope of Practice Disclosure: I only practice massage within the above-defined scope of practice for a Florida licensed massage therapist. My charges are for services rendered under the Florida scope of practice of massage therapy. Nothing else.
I may teach you self-care stretching techniques as allowed under my scope of practice. I refer to these as “stretches”. I am not allowed to teach you “exercises” with the intent of rehabilitating muscle or muscle building. The only “exercise” I may teach you will be to induce proper muscle tone to maintain posture.
I may recommend other ideas which may enhance your health, but please understand that these are personal opinions, not professional recommendations. I will only give these opinions if I see that you are open to receive them, however; you have the right to refuse any of them or stop me from even suggesting them. These health recommendations are given freely (without charge) and are not a part of your massage therapy.
Scope of Practice Disclaimers: I make these disclaimers so that there is no confusion about me:
I disclose that am not a “Doctor” or physician, nor should you address me as one. I do not practice medicine, osteopathy, podiatry, naturopathy, chiropractic, or acupuncture. I am neither a licensed physical therapist nor a licensed occupational therapist (even though some of these therapists’ scopes of practice do overlap with my own). If you need clarification in this area, please ask.
I do not diagnose, treat (except as stated below), operate, or prescribe for any disease, pain, injury, deformity, or other physical or mental condition. I will treat, within the Florida scope of practice for massage therapy, a disease or medical condition that has been diagnosed by a state-licensed physician as prescribed by that physician.
Otherwise, I make no claim that I can treat or cure any disease or medical condition.
Also, I make no claim as to be an expert in any field, nor should you believe I am one.
My primary claim is that I facilitate health so that your own body may heal whatever may ail you. I define “health” as the optimal state of being or function. I typically evaluate structural health by a postural evaluation during the first session, but any information I may share with you from that should not be considered a medical diagnosis.
Except as explained above, my massage therapy sessions should only be construed as to: Improve health and quality of life, provide for a means of preventative healthcare, relieve muscle spasm, restore elasticity and circulation to soft tissue, and provide an effective means to relieve stress (as per Gov. Jeb Bush declaration October 24, 2004).
Type of Business Disclosure: I am a renter at Natural Approach Acupuncture & Massage Center. Any therapeutic relationship for massage therapy is between you and me. Any financial relationship for massage therapy services is between you and me.
Right of Refusal: As a business operator, I reserve the right to refuse service to anyone for any reason by giving you proper notice as defined and allowed by Florida Statutes
